Your choice, every step.

Medicare patients aren’t required to do anything. If your primary care doctor participates in our ACO, Medicare may automatically include you so we can coordinate your care. You can also actively choose us — or opt out at any time.

You can also opt out of having your Medicare claims data shared with the ACO. This will not affect your care, your benefits, or your relationship with your doctor.

How to opt out

  1. Call 1-800-MEDICARE (1-800-633-4227), 24/7.
  2. Tell the representative you’d like to opt out of having your data shared with an ACO.
  3. Or send a written request to your ACO compliance officer (see Contact).

Opting out is free and never affects your care or your Medicare benefits.

Do I have to enroll? Will my insurance change?

No. You don’t enroll in anything, and your insurance does not change. You stay in Original Medicare. The ACO simply gives your doctors better tools to coordinate your care.

Yes. You see the same primary care provider and specialists you choose today. You can also see any Medicare provider, anywhere in the country, just like you can now.

No. There are no premiums, copays, or fees for being part of an ACO. Your Medicare coverage and costs work exactly as they did before.

The LEAD Model is a Centers for Medicare & Medicaid Services program for new and growing ACOs. It encourages physician-led organizations to take responsibility for the quality and total cost of care for the patients they serve.

Your data is protected by HIPAA and additional CMS rules for ACOs. Information is shared only with your care team to coordinate care. You can opt out of having your claims data shared with the ACO at any time by calling 1-800-MEDICARE.
